• Persons who live in Logan, Morgan, Phillips, Sedgwick, Washington and Yuma Counties.
• Persons whose primary physician practices in these counties.
• Each patient must meet the admission requirements as set forth by Federal regulations.
• Services are provided regardless of race, national origin, religion, gender, sexual orientation, age, disability, terminal diagnosis, social status or ability to pay.
