How to Receive Hospice Care

• Physicians, family members, or patients may call Hospice of the Plains to request hospice care.
• A nurse will return the call, collect the necessary information, then contact the primary physician to ensure hospice care is appropriate.
• An initial home visit will be scheduled to explain hospice services and answer any questions.
• Admission to hospice may be done at that time or whenever a patient chooses.
